Health Assessement Requirement:
Beginning in the 2016-2017 school year, State law requires that every child entering public schools in N.C. for the very first time; receive a health assessment (check-up/physical). The assessment must occur within 12 months prior to entering school. The medical provider, parent or guardian must provide a completed health assessment transmittal form to the school upon enrollment.
Who will this affect?
• All Kindergarten students
• Any student (K-12) moving from another state and enrolling in a NC public school for the very first time
• Any student (K-12) previously homeschooled and enrolling in NC public school for the very first time
• Any student (K-12) previously attending a private school in NC that did not require a NC health assessment and the student has never been enrolled in a NC public school
North Carolina law requires all children in the state to receive certain immunizations. Records are checked when children enter school
