Elementary School Information

Within the elementary schools the nurse's are busy doing many things.  Below are just some of the few things that our nurse's do within in their school during the year.

 Vision Exam Chart  
  • Vision Screening for 1st, 3rd, and 5th graders and by request from teacher, parent, or student.
  • Dental Screening for Kindergarten and 5th graders.  This screening is done by the state Dental Hygienist.
  • Colleberate with State Dental Hygienist for the Give Kids A Smile Program in February.
  • Assist with administration of Fluoride Mouth Rinse in selected schools.
  • Height, Weight, and BMI screening for 5th grade. 
  • Care plans for chronic health conditions
  • A variety of health education classes are taught throughout the school year.
  • EC screenings are also performed by the school nurse upon request from the EC teacher.
  • Hearing will be checked by school nurse upon request of teacher's and if child fails hearing screening done by school staff.
  • The school nurse will be coordinating with the staff at your child's school to assist and schedule hearing screening for all 1st Graders.
 Dental

2011-2012 Kindergartners

  • Childern must be born on or before August 31, 2006 to  start kindergarten in the 2011- 2012 school year.
  • Health Assessment form (blue physical form)  completed by health care provider.  Physical must have been on or after August 25, 2010.  You can get these forms from the school or at your health care providers office.
  • Your child may start school without their physical or immunizations. However,  you only have until the 30th calendar day from when school started to get these completed.  This year the day is September 23, 2011.  After that day your child MAY NOT be able to attend school until these are completed.
  • All immunizations must be up to date and you must bring a copy of your child's immunization record. This record must include the doctor's signature and/or be stamped by the clinic that gave the shots.
  • Each child must have a vision and hearing screening as part of the kindergarten check up.
  • The school will also need a copy of your child's Social Security Card.
  • Two proofs of residence are required- such as a power bill and driver's license.
  • A certified copy of your child's birth certificate.

Rising Sixth Graders

  • A booster dose of Tdap vaccine is required for all children before entering 6th grade that are enrolled in public school or by the age 12 for those not attending public schools on or after August 1, 2011.
  • A booster dose of Tdap vaccine is also required if it has been five or more years since the last dose of tetanus-containing vaccine.
  • What is Tdap?  It is a vaccine that has a combination of three vaccines that provides protection against tetanus, diphtheria, and pertussis (whooping cough).
  • This new law went into affect January 1, 2008.
  • Your child's school nurse will need proof of immunization before the 30th calendar day from when school starts.  This year the day is September 23, 2011.  After that day your child MAY NOT be able to attend school until these are completed.
