All students with disabilities by the age of 14 must have a transition component/plan as part of their Individualized Education Program (IEP). The transition process provides a coordinated set of activities designed with the goal of promoting successful movement from school to post-school activities, including post-secondary education, vocational training, integrated employment, adult services, independent living, and community participation.
The Individuals With Disabilities Education Act addresses transition as follows:

Beginning at age 14, and updated annually, a statement of the transition service needs is required.

Beginning at least one year prior to the child reaching the age of majority, a statement that the child has been informed of his rights must be completed. |
Additional Required Statements:

A statement of the needed transition services beginning no later than age 16.

A statement of interagency responsibilities.

A statement noting that should a participating agency fail to fulfill their agreed upon responsibilities, the school shall reconvene a meeting of IEP participants to identify alternative strategies.

If a component of the transition plan is deemed not needed, a statement to that effect must be attached to the IEP. |
